1. Open, download the digital DEE CRF,
2. record, save your data
3. and e-mail your completed digital DEE CRF to m.witteveen@procdm.nl

You'll receive your data in return (MsExcel file).

You need the free Adobe Reader version 9 or X to be able to complete a digital DEE CRF. (http://www.adobe.com/downloads/)

Digital DEE CRFs contain soft data checks to help you during CRF recording. To ensure capture of clean data, Digital DEE CRFs also contain hard data checks, executed just before you sign a Digital DEE CRF for completion. Data checks to ensure data collected within Digital DEE CRFs is as clean as possible with a CRF visit.

Please save and lock your data for completion by signing with your digital signature. Click on the Investigator signature field at the end of each Digital DEE CRF, and add your digital signature. The first time you will be asked to create your personal digital signature.
In case you want to save your recordings earlier, e.g. for future completion, go to File —> Save As (or CTRL+SHIFT+S).

It is recommended to save your file with the file name proposed in the Filename for saving field, which you can find just before the Investigator Signature at the end of each Digital DEE CRF. (You are recommended to copy/paste the proposed file name.)
